Clinical Anatomy of the Physical and Chemical Threats of Volcanic Ash to the Human Body

Volcanic ash represents a medical and geological hazard that is structurally distinct from combustion residues such as household dust or organic debris. Composed of fragments of magmatic glass, crystalline silica, and minerals produced by high-pressure fragmentation, volcanic ash has sharp, abrasive, and acidic surfaces capable of causing tissue injury across multiple body systems. This review synthesises clinical, toxicological, and epidemiological literature on the pathophysiological consequences of volcanic ash exposure affecting the respiratory, ocular, dermatological, and gastrointestinal systems, with particular attention to systemic fluoride toxicity. Acute mucosal abrasion, inflammatory bronchospasm, corneal epithelial injury, irritant contact dermatitis, and chronic silicosis are discussed as key clinical manifestations, alongside sociological dimensions of risk perception and disaster preparedness in volcanically active regions such as Indonesia. Protective measures, including properly fit-tested N95 and FFP2 respirators, goggle-style eye protection, and non-friction irrigation techniques, are evaluated based on empirical evidence of filtration efficiency. The discussion situates medical responses within broader geological and public health contexts, emphasising that effective volcanic ash mitigation requires interdisciplinary coordination among clinicians, geologists, and public health authorities. Findings indicate that ash toxicity is influenced by particle size, crystalline silica content, and surface acidity, all of which vary between eruptions. Preventive medical protocols, culturally sensitive risk communication, and continued toxicological research remain essential for reducing morbidity among populations living near active volcanoes.
